• Students who have completed their internship (including those eligible for graduation) must submit their internship notebooks to the commission member within three (3) months after the internship end date.

• Internship notebooks not submitted within this period will not be accepted, and students will be required to repeat their internship.

• This document must be printed from HERE and signed by commission members.

• If internship places request online applications, the document can also be sent via email.
